I came across this video demonstrating how to make a glue bound hardcover using a small professional binding machine. I've seen similar binders for sale on eBay for around $1,500 US. I've been wanting to get one of these - just can't justify the expense for my little ebook print outs.
The same machine does paperbacks too.
